The MSDE Office of Child Care requires that a blood lead assessment be completed by your child's pediatrician prior to enrollment.

Age-appropriate immunization requirements for licensed childcare providers are based on COMAR regulations. Please have this form reviewed and completed by your child's pediatrician. This also must be completed prior to enrollment.

The MSDE Office of Child Care Emergency Contact Form must be completed prior to enrollment.

The MSDE Office of Child Care Medication Administration Authorization Form must be completed in order for child care providers to administer ANY medications including over-the-counter meds.
This form must be completed by your child's pediatrician and returned prior to enrollment.
